Organization - OMHA COVID-19 Vaccine Policy Announced
Submitted By Stacey Dineen on Sun 09 05, 21
The OMHA announced that all participants (players and volunteers) born in 2009 or earlier must be fully vaccinated for COVID-19 by no later than October 31, 2021.

All coaches, trainers, volunteers, and players are to email their COVID-19 Dose Administration Receipt or Medical Record to the NHHA Privacy Taker at [email protected] ASAP.

Failure to provide proof of vaccination by October 31, 2021 will prevent players and volunteers from entering the arena, playing on a team, or being rostered as a volunteer until documentation is provided.  Participants can apply to OMHA for special accommodation, details to be provided soon.

Anyone wishing to de-register may do so by emailing [email protected] by no later than October 1, 2021 to receive a full refund.  Refunds will not be provided after October 1, 2021.

For more details please refer to the OMHA article (https://www.omha.net/news_article/show/1179916) and OMHA Vaccination Policy (https://mcusercontent.com/ed1fb6c79cb1b8204b16b527d/files/62795759-e762-cbbf-572f-38384f7aca4c/OMHA_VACCINATION_POLICY_V1_2.pdf)

Public Health and Wilmot Recreational Facility may have their own requirements and timeline for spectators (including parents and/or guardians) to be vaccinated to enter the facility.
