2016/17 Coaches Application
COACHING CREDENTIALS AND CODE OF CONDUCT

I agree that I will upgrade my credentials as requested by NHHA or my application will not be considered.  If selected I will attend all coaches meetings and comply with all coaching requirements as outlined in the Constitution and By-laws of the NHHA and the Ontario Minor Hockey Association (OMHA).
I agree that I have read and understood the Code of Conduct and Team Official Job Descriptions that are posted on the NHHA website.

SPECIAL REQUIREMENT FOR NEW COACHES TO NEW HAMBURG HOCKEY

All coaches new to NHHA are required to submit a complete SEASONAL PLAN including: skill & drill examples, tournaments, fundraising, off ice training and Coaching philosophy with respect to players/parents and staff members, at their interview.

Coaches who have been approved to coach in the previous two seasons within the NHHA organization are NOT required to submit a Seasonal Plan, however may be requested to provide one once the staff has been selected. 
 
 

POLICE CHECK - MUST accompany coaches application in order to be considered for any position applied for, if you do not have a current (in past 3 years) police check do not complete this application until you have obtained one

I understand that as a Coach or team volunteer, NHHA and OMHA require that I have a Valid Police Records Check (good for 3 years from issued date) on file with NHHA. 

Police Record Check - print off a Police Record Check Confirmation form under coaches tab and take to Headquarters in Cambridge and you will receive it the same day - This can then be scanned and emailed to [email protected]

Coaches of House League/Local League/ Minor Development / Rostered Select in divisions below Novice (Initiation & Tyke) must hold Coach 1-Intro to Coach qualification - No other qualifications are accepted 2.
